Any NorthCreek Church sponsored event where students (minors) are driven by anyone other than their own parents or legal guardian (like in a carpool) requires a completed permission slip signed by their parents or guardian.
This permission slip allows students to participate in the event, and allows the sponsors, leader or adult chaperones of a Flock event to administer first aid and/or transport the individual to the nearest doctor or hospital for further medical attention. This form is for your student's safety! Without it, a doctor will not give them medical attention, unless the injury is immediately life threatening. It also releases NorthCreek Church, its paid staff, and any adult volunteers helping out, from legal liabilities in the unforeseen event of a health or safety issue involving their son or daughter.
If you are okay with the risk, there are two permission slips available for your use.
One is an Annual Permission Slip. This document is used to save parents/students time in filling out the same information (that hasn’t changed) every time for each event.
PLEASE NOTE: If any of your information changes, we will need an updated form so that your child is assured of proper treatment, should the need arise.
This document will be kept on file by Student Ministries (for a specified period of time) and will be used if a problem or an accident occurs.
There is also a One-time Permission Slip that is used by students who don’t participate on a regular basis. This document is not kept on file and a new permission slip will be needed for you to participate in another NorthCreek sponsored event.
Occasionally an organization or facility that we use requires their own Release of Liability form.
Students desiring to participate in an event that requires a permission slip or release form must submit a completed one before they can participate.
