Attention All Drivers:

Thank you so much for volunteering!

NorthCreek Church requires the following items
to be submitted by our volunteer drivers:

1) Insurance Verification Form
& Volunteer Transportation Agreement

(a new form is to be filled out for each event
that a volunteer is driving for)

2) Copy of Drivers License

3) Copy of Insurance ID Card
(showing effective dates of insurance coverage)

4) Copy of Insurance Declaration Policy
(showing limits of insurance coverage)

*Please copy each document to a seperate piece of paper
because expiration dates are different for each item.
 


*Item 1 will be discarded
after the event that it was filled out for.

*Items 2, 3, and 4 will be kept in the Student Ministries Office
until they expire. You will be reminded by Student Ministries
to update documents that will soon expire.



Please drop-off, mail, or fax these required documents to NorthCreek Church before you drive on a church event. It is required by law that all drivers have the required items on file.
