Vehicle Drivers:
Before you can drive students in your vehicle
to and/or from a NorthCreek Church sponsored event,
we require that you complete a
Volunteer Driver Qualification Form

If you would like to receive compensation for the fuel purchases you make for this event, please put a check mark in the appropriate place on this form. We will AUTOMATICALLY send you a reimbursement check within 2 weeks.
Please DO NOT submit VEHICLE fuel receipts. Just place a check mark on this form.
You will be reimbursed at the I.R.S. mandated rate of 0.14 cents per mile.
Submit this form to the Student Ministries Office before you leave for the event!
*A new form must be filed out for each event that you volunteer to drive for

Please note that the law has changed regarding life jackets from 12 to 13

during a NorthCreek Church sponsored event,

NorthCreek Church Watercraft Policy form

Submit this form to the Student Ministries Office
before you leave for the event!
If you would like to receive compensation for the BOAT fuel purchases you make for this event, please submit all BOAT ORGINAL fuel receipts to the Student Ministries office no later than 30 days after the event (no VEHICLE fuel receipts, please; for more information on vehicle fuel reimbursement, please see above).
