Permission Slips & Release Forms

To the left of your screen (under the "Permission Slips & Release Forms" tab), you will now find two permission slips that are available. Choose the appropriate one for your situation, print it off, fill it out completely and bring it with you as you drop off your son or daughter for an event.

But before you do that, please be aware of the following:

Parent's are ulimately responsibile for providing NorthCreek Church staff with the most current information for their son or daughter on any given event. 

Any NorthCreek Church sponsored event where students (minors) are driven by someone other than their own parents or legal guardian (like in a carpool) requires a completed Permission Slip signed by the student's parents or legal guardian.

A Permission slip also gives us your approval, for your son or daughter to participate in an event and releases NorthCreek Church, its paid staff and any adult volunteers helping out, from legal liabilities in the unforeseen event of a health or safety issue involving your son or daughter.

Please understand that we try our best to make good decisions for the welfare and safety of every participating student, but we can and do make mistakes. There are also things beyond our control that can occur and so we cannot guarantee the safety and welfare of any student. This is a risk that each parent needs to evaluate and decide upon. If you are not comfortable with this risk, and you want your son or daughter to participate, then please accompany them to the event and be involved. Otherwise, don’t allow your son or daughter to participate.

If you are ok with this risk, there are two permission slips available for your use.

One is an Annual Permission Slip. This document is used to save parents/students time in filling out the same information (that hasn’t changed) every time for each event.

This document will be kept on file by Student Ministries (for a specified period of time) and will be used if a problem or an accident occurs.

Parent's have the responsibility to ensure that NorthCreek has the most current information for their son or daughter on any given event.

There is also a Single Event Permission Slip that is used by the parents of students who don’t participate on a regular basis. This document is not kept on file and a new permission slip will be needed for their son or daughter to participate in another NorthCreek sponsored event.

Occasionally an organization or facility that we might use requires their own Release of Liability. You would then need to print out, fill it out and bring it with you as you drop off your student.

Students desiring to participate in an event that require a permission slip or release form must submit a completed one before they can participate.
