Without brothers and sisters like you volunteering, we would not be able to take our students to off-site events. 

In an effort to help keep our students safe and to minimize your legal risk and that of NorthCreek Church, it is necessary that you read over our Volunteer Driver Qualification Form and make sure that you qualify to be one of our drivers.

Please complete any info needed on the form, then sign and date it. Just bring it with you and give to Lee when you show up for the event. We'll keep it on file until everything is completed and then discard it.

Lee will have blank copies available of this form at our meeting spot, just in case you forget to fill one out or if you have trouble printing one.
